Q1 Who is eligible for the program?
  Allen EAP services are available to UNI employees who qualify for UNI insurance benefits, as well as their immediate tax dependant family members and/or those family members covered on your health insurance plan

Q2 How do I contact Allen EAP?
  To schedule a confidential appointment, call (319) 235-3550, or toll free at 1-800-303-9996, Monday through Thursday, from 8:00 a.m. to 8:00 p.m. and Friday, from 8:00 a.m. to 3:00 p.m. Evening appointments are available. Identify yourself as part of your organization's Employee Assistance Program. We'll schedule an appointment to meet your needs.

Q3 What if it’s an emergency?
  If a personal crisis occurs when our office is closed, call (319) 235-3550. The answering service will page an EAP counselor who will call you within one hour of your call to help you stabilize the situation, and will follow-up with you to schedule an appointment if you need further support.

If you need immediate medical assistance, the counselor will assist you in obtaining services outside the EAP, such as a doctor or an Emergency Room.

Q4 Is the EAP confidential?
  All contacts made by employees and family members to the EAP are confidential. Your participation in the program will be treated confidentially in accordance with all state and federal mandates.

Q5 Does the EAP cost anything?
 

EAP services are paid by your organization. The university pays an annual contracted fee for this service regardless of utilization. EAP services cover visits with an EAP counselor for the initial problem assessment and short-term counseling. If you and your counselor decide medical assistance or additional counseling is needed, your counselor will refer you to another agency. If referred, you will be responsible for any costs not covered by insurance.


No one lives a life without problems. When a personal problem disrupts your work or relationships, we encourage you to use the Employee Assistance Program.

Q6 Are both male and female counselors available?
  Currently (as of April 2010), all counselors at the Allen site are female. If you would prefer to see a male counselor, Allen EAP can supply a voucher to see a male counselor in Cedar Falls. To schedule an appointment, you would still call the Allen EAP office at (319) 235-3550 or toll free at 1-800-303-9996 and make a request for a male counselor.

Q7 Where is Allen EAP located?
  The Employee Assistance Program is located on the 5th floor of Allen Hospital in Waterloo. The offices can be found by taking the “B” elevator to the 5th floor of the main hospital. Directional signs for “B” elevator will be easily seen at either the front lobby entrance on Logan Avenue or at the admission entrance on Donald Street. Parking options are available at both of these entrances. As soon as you step off the “B” elevator, you will see the waiting room area and receptionist’s office. Please call Kendra, the office manager, at 235-3550 if you need any additional assistance with directions.
