There are two types of performance appraisal forms for Merit employees.

1. The Merit (AFSCME-Covered) Employee Annual Performance Summary is for organized contract-covered employees.  

2. The Supervisory & Confidential Employee Annual Performance Summary is for non-contract, non-bargaining unit employees.   

An optional Discussion Guide for Annual Performance Summary,   to be completed by the employee, may also be used with either evaluation form.  For assistance with conducting performance appraisals, please refer to the Supervisor Guidelines for Conducting Employee Performance Appraisals.
